2024-2025 New Student Acceptance Form

Welcome to the St. Theresa Catholic School family!

•Please complete this form to enroll your child(ren) for the 2024-2025 school year.

•If you are a new school family, please complete the FACTS Tuition Payment registration online within 48 hours.

•If you are a current school family, your new incoming student will be added to your current FACTS account.

The $300 non-refundable enrollment deposit will be deducted through FACTS for all new students. It is part of tuition, not an additional charge.

Kindergarten students will be charged a $70 supply fee which will be deducted through FACTS. Grades 1-8 will be responsible for purchasing school supplies on their own. Supply Lists will be posted on the website this summer.

PAYMENT INFORMATION (if enrolling)

Paying in full through FACTS by June 1, 2024 and receive 2.5% discount.

Paying in twelve monthly payments through FACTS beginning July 2024 and continuing through June 2025.

Below are the policies regarding fees, deposits and withdrawing a student. By checking the boxes below you are agreeing to adhere to school policies regarding refunds and payments.

Enrollment Deposit *

The Enrollment deposit is $300 per child.

Withdrawal Policy for St. Theresa Catholic School *

For students withdrawing once school has begun, the current month's tuition will be forfeited. If the tuition was paid in full, the remaining tuition will be refunded once outstanding fees have been paid.

For students who have been accepted and enrolled, but decide to withdraw before August 15, 2024, a 50% deduction will be applied to tuition refunds. (If a family has paid the full tuition at this time, then the 50% deduction will apply only for two months, and the remainder will be fully refunded.)

For students who have been accepted and enrolled, but decide to withdraw after August 15, 2024, only the tuition paid during the month of September or later will be refunded on a prorated basis. Tuition received for July and August and the completed months of attendance will be forfeited.

If tuition is paid in full by June 1, 2024, there will be a 2.5% discount.

All families are required to provide 16 hours of service per year. Families have the option to buy out.
