I/We the undersigned have legal custody of the student mentioned above, a minor, and hereby give my/our consent for him/her to attend events being organized by Pride of Texas Music Festivals LLC. I/We understand that there are inherent risks involved in the attractions included in the package and while performing at the festival using the festival equipment at the participating venue.
I/we hereby agree to release Pride of Texas Music Festivals LLC and its employees and agents from any and all liability for any injury, loss, or damage to person or property that may occur during the course of my/our child’s involvement in the attractions included in the package and while performing at the festival using the festival equipment at the participating venue.
In the event my/our child requires treatment for any reason from a physician and/or hospital personnel designated by the school’s sponsors, I hereby consent to medical treatment as deemed necessary by a licensed physician, and I/We agree to hold Pride of Texas Music Festivals LLC free and harmless of any claims, demands, or suits for damages arising from the giving of such consent and/or treatment. I/We realize that I/We will ultimately be responsible for the cost of any medical care and I/We acknowledge that Pride of Texas Music Festivals LLC does not assume any financial responsibility for any costs of medical care and I/we agree to hold Pride of Texas Music Festivals free and harmless for said costs.
I/We have read and are familiar with the School’s Rules of Conduct with which my/our child will be expected to comply.
