This application must be received by August 31th, 2018


This application form is needed in order for the insurance company to provide insurance coverage for your stored items/contents. The contents must be located at the address provided unless it is being used for official corps business (IE camping trip, official activity etc). We do NOT require a list of all equipment, however we require information about the storage location (see below).
