UGA/OGE Travel Registry Proxy Access Request Form

This form is for requesting access to be able to submit International Travel Registrations on behalf of another UGA traveler.

Important notes:

If you have submitted a travel registration on someone else's behalf before, you do not need to request access. Existing travel registration proxy users are "grandfathered" in.
Access requests are processed Monday - Friday, 8:00 am - 5:00 pm (EST).
