The health and safety of each of our guests is very important to us. Vaccination record must be presented prior to grooming. All pets must have current vaccination on the following:   
  • Rabies
  • Distemper / Parvo Virus
  • Bordetella
  • Canine Influenza H3N8
  • Canine Influenza H3N2
***ONLY PETS WITH CURRENT VACCINATION WILL BE ACCEPTED FOR GROOMING***

Our business hours are Monday thru Sunday from 11:00AM - 7:00PM. We'll take the last appointment at 4:00PM. Please note that no appointment requests are confirmed until you hear back from us. If you do not hear from us within the next 30 minutes, we encourage you to call us at (702) 658-8820.

RELEASE INFORMATION (please initial each section)

Accidents:  Every effort will be taken to make sure your pet gets groomed as safely and carefully as possible.  But, unfortunately, ACCIDENTS DO HAPPEN. Dogs are wiggling, scratching, licking, jumping creatures. Whereas dog grooming tools, by necessity, are sharp instruments. In case of mishap, Puppy Boutique Las Vegas will contact you immediately, but will take necessary measures to get your pet the medical attention he/she requires. In the event of emergency, Puppy Boutique Las Vegas will transport your pet to a veterinarian and that any resulting bills will be your sole responsibility.
Matted Dog Policy:  Pets with matted coats require extra attention.  If matting is minimal and the pet responds well, we may demat for an extra fee. However, if the matting is severe the least painful and stressful way to remove the mats is by shaving the dog. There is a greater risk of nicking and scratching a matted dog during the grooming process.  In addition, the skin may appear red, itchy, and irritated due to lack of oxygen reaching in and under the mats. Puppy Boutique Las Vegas will not be held responsible for any injury sustained while grooming a matted dog, including any after effects.  This includes, but not limited to swelling, redness, itchiness, and self-inflicted abrasions due to external rubbing.
Flea & Tick Policy:  If your pet has fleas,  we will automatically administer a flea bath.  We will notify you if possible.  The charge for this service is $10. This cost covers the shampoo, time, and cleaning necessary to ensure that no fleas are active in our premises.  The treatment will kill the fleas on your pet, but after leaving our premises, your pet will not be protected from any fleas that may be present in your home or other area.
No Shows/Cancellations:  Please give us a least a 24-hour notice if you cannot make it to your appointment.  The first time you are a no call/no show we will add a $20 charge to your next grooming appointment.  After the second no call/no show you will be required to prepay for all your future appointments.
Late Pick-up Charges:  We DO NOT have boarding services at Puppy Boutique Las Vegas. If you are late in picking up your pet you will be charged $1.00 per minute for every minute that you are late after our regular business hour. NO EXCEPTIONS.
Customer Complaints:  If you are unhappy with our service you must notify us within twenty four (24) hours.  We will do what we can to remedy the situation. If there is some kind of miscommunication, we will take note so that we can make it better next time. THERE WILL BE NO REFUNDS NOR DISCOUNTS.
Right To Refuse Service: Puppy Boutique Las Vegas reserves the right to refuse service to customers whose pets may pose a threat to our employees or other pets in our care.  This includes aggression problems, health problems and parasite problems.
I am the owner or a representative of the owner of the pet described above, and I have the authority to execute this consent. By signing below, I understand and agree to release and hold harmless Puppy Boutique Las Vegas, its owners, employees and affiliates from and against any and all liabilities, expenses, damages, and costs (including attorney fees) resulting from any service provided or injury (including death) to your pet(s) while in our care or afterward.
