Grooming Consent and Release Form

The health and safety of each of our guests is very important to us. Vaccination record must be presented prior to grooming. All pets must have current vaccination on the following:

Rabies
Distemper / Parvo Virus
Bordetella

***ONLY PETS WITH CURRENT VACCINATION WILL BE ACCEPTED FOR GROOMING***

Our business hours are Monday thru Sunday from 11:00AM - 7:00PM except Tuesdays. We'll take the last appointment at 5:00PM. Please note that no appointment requests are confirmed until you hear back from us. If you do not hear from us within the next 30 minutes, we encourage you to call us at (702) 658-8820.
