of a completed course, located in the individual course classrooms.

Remember: Athena’s & AHA do not maintain records of student enrollment after the culmination of the current school year.

This is not the date of the courses. This is the date you would like for the Report itself. The Report's date may be specialized. For example, if the courses were taken in another year, we can post-date the Report if needed. *

Course Names and Course Semester.

List the titles/names of the courses your student took at Athena's or AHA. The courses listed should correspond to the organization chosen above (Athena's or AHA).

If a grade is desired, be sure to include the desired grade. Remember, since neither Athena's nor AHA are schools of record, we can only list a **recommended** grade on the report. This report does not constitute a transcript.

Please place each course name and its information on a new line.
